About the Program
Primary care service provides diagnosis and treatment for illnesses and other health issues with an emphasis on health care prevention. Primary care includes an extensive range of treatment modalities with emphasis on health care prevention.
VHA primary care gives eligible Veterans easy access to health care professionals familiar with their needs. It provides long-term patient-provider relationships, coordinates care across a spectrum of health services, educates, and offers disease prevention programs. Primary care now serves as the foundation of VHA health care and has become the first point of contact with the health care system for Veterans enrolled in VHA.
Since 2009, VHA has been engaged in transforming the primary care system into a new model of health care delivery that is a patient-driven, team-based approach based on the Patient Centered Medical Home model. This care is delivered in teams, called Patient Aligned Care Teams (PACTs), and is based on principles that promote effective, efficient, comprehensive care through continuous communication and improved coordination of services throughout the health care system.

• Chronic obstructive pulmonary disease (COPD) treatment
• Diabetes prevention and treatment
• Flu vaccine and nasal mist
• High blood pressure prevention and treatment
• Treatment for obesity
• Osteoporosis treatment
• Prostate disease prevention and treatment
• Smoking cessation
